When completing your Direct Service Activities in the OnCorps system, please include the following required information:
Name of K-12 School where you are student teaching using service learning
Full Mailing Address
City, State Zip Code
Contact Person (Host Teacher, for example)

Exiting the Program
To exit the program, you need to complete the online exit survey and print out, sign and mail the exit form. Submit an electronic copy of your service learning lesson plan to your campus supervisor.

Education Award Information
For general questions about the AmeriCorps Education Award and student loan deferment, contact National Service Trust at 1-888-507-5962 or edawardvoucher@cns.gov. Operators are available Monday through Friday from 7:00 am to 8:00 pm EST.
If you have successfully completed your AmeriCorps service, you can view your AmeriCorps Education Award account activity online. To request a copy of your Education Award voucher or report a change of address, you can e-mail edawardvoucher@cns.gov.
